I've been reading the posts about providing Social Security numbers when applying for MS companies. I've only applied to companies listed on Volition's list of MS companies, and so far I've provided my SS number on each application that requested it. I thought it was okay since I've only applied to the companies noted to be reputable by Volition, but now I'm starting to feel a little uneasy about it.

I'd like to go back and remove my SS number from my profiles, if possible, or provide them with an Employer Identification Number instead. I've been to the IRS website and have taken a look at the online application for an EIN.

I'm hoping someone who has already done this can help me fill out the application. Here is the online application: https://sa1.www4.irs.gov/sa_vign/newFormSS4.do

There are a few questions in particular I'm not sure how to answer.

2. Trade Name of Business
Is this simply Mystery Shopping, since I have no business name?

3. Executor, trustee "care of" name
Is this my own name?

7a. Name of principal officer, general partner, grantor, owner, or trustor
My own name?

8a. Type of entity
Sole Proprietor?

11. Closing month of accounting year

Any other information about applying for an EIN would be appreciated!

#2 - not required - only if different from your name (if you were using a DBA)

#3 - not required

#7a - not required

8a - yes, you are a sole proprietorship

11. - December
